On Line Bill Submission

On line bill submission will help us to process contractor’s bills more quickly and with fewer chances for error. Please fill out the form on the following page as completely as possible.

After submitting your bill by clicking on the button marked ‘Enter Information’ at the bottom of the page, please print your bill, sign it and either mail it or fax it to the office.
No checks can be mailed until we receive the signed form.
Please print a second copy for your records.
